We are seeking a highly motivated and dedicated Registered Nurse to join our Primary Care Team in Fort St. James. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities
- Develop and implement care plans in collaboration with the interprofessional team
- Conduct initial and ongoing client assessments and provide nursing care through appropriate interventions
- Communicate effectively with clients, families, and healthcare professionals
- Participate in team huddles and other activities to promote communication and collaboration
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid driver's license
- Experience developing and providing formal education to patients, families, and the general public
- Knowledge of adult learning principles and facilitation skills
- Ability to operate related equipment, including relevant computer applications
