We are seeking a highly skilled and dedicated Registered Nurse to join our Primary Care Team. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and provide formal education to patients, families, and the general public on various health topics.
- Work collaboratively with the interprofessional team to provide comprehensive care and services to individuals, groups, and communities.
- Participate in shift rotation and work hours of 08:30 to 16:30.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
- Comprehensive benefit packages, including MSP, extended health/dental, and a municipal pension plan.
- Four weeks vacation with one year of continuous service.
