Primary Care Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our Primary Care team in Fort St. John. As a Primary Care Nurse, you will provide high-quality patient care and work collaboratively with our interprofessional team to deliver exceptional health services to our community.
Key Responsibilities:- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Conduct health assessments, develop and implement care plans, and provide education to patients and families.
- Collaborate with community partners, including physicians, nurse practitioners, and other healthcare professionals, to provide comprehensive care.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
