About the Role:

Closing the Gap Healthcare Group is seeking a highly skilled and compassionate Care Team Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of exceptional client-centered care.

Key Responsibilities:

  • Coordinate visits and communication with our healthcare providers to ensure seamless care delivery.
  • Provide empathetic client service and administrative support to maintain accurate records and ensure efficient operations.
  • Schedule and coordinate service providers to meet client needs, utilizing strong problem-solving skills to address any challenges that may arise.
  • Effectively triage calls to ensure effective call management and provide support and information to internal and external customers.
  • Perform data entry requirements for all clients using our computer system, maintaining precise attention to detail and efficiency.

Requirements:

  • Outstanding commitment to providing excellent client-centered customer service.
  • Effective verbal and written communication skills, with the ability to interact with clients and healthcare providers in a professional manner.
  • Precise attention to detail and efficiency in data entry, with knowledge of medical terminology an asset.
  • Computer and phone literacy, with the ability to work effectively in a team-oriented environment.
  • Diploma from a Community College in Business Administration, Office Administration, or Medical, or Office Administration and/or Grade 12 Diploma with related experience.
