Job Title: Care Coordinator

About the Role:

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.

Key Responsibilities:

  • Collaborate with patients and their families to assess care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, respecting their values and preferences
  • Balance patient needs with available resources, ensuring effective care delivery

Requirements:

  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills
  • Established ability to complete required documentation, reports, and forms
  • A valid driver's license and access to a reliable vehicle
  • Proficient in a Windows environment

Preferred Qualifications:

  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
