As a Palliative Care Coordinator, you will play a vital role in coordinating the care and management of palliative care patients in the community, ensuring they receive the highest level of care and support.
Key Responsibilities- Assess Care Needs: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans for the complex population.
- Link Patients with Community Services: Connect patients with community service providers to maintain the patient's safety in their own home while prioritizing the prevention of hospital admission or ED visits.
- Coordinate and Monitor Care Plan Delivery: Coordinate and monitor care plan delivery, ensuring patients receive the necessary support and services to meet their needs.
- Establish a Helping Relationship: Establish a helping relationship with patients and their families, providing emotional support and guidance throughout the care process.
- Act as a Subject Matter Expert: Serve as a subject matter expert for colleagues and external partners with respect to palliative care needs of patients in the community.
- Membership in Good Standing: Membership, in good standing, with an applicable regulatory body as a Registered Nurse (RN, BScN).
- Experience in Community Health: 1+ years of recent experience in community health or a related field.
- Training/Certification in Palliative Care: Training/certification specific to palliative care (i.e., Fundamentals, LEAP, CAPCE, etc.).
- Knowledge of Palliative Care: Knowledge of EDITH protocol, Symptom Response Kits and DNR-C paperwork, and PPS Scale.
- Strong Clinical Background: Possesses a strong clinical background with a patient-centered approach.
- Knowledge of Healthcare Delivery System: Knowledge of the healthcare delivery system and community resources.
- Leadership and Advocacy Skills: Is a natural motivator, leader, and advocate.
- Excellent Interpersonal Skills: Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Time Management and Organization: Effective time management, prioritization, and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment.
- Documentation and Reporting: Established ability to accurately complete required documentation, reports, and forms.
- Driver's License and Vehicle: A valid driver's license and access to a reliable vehicle.
- Proficiency in Windows Environment: Proficient in a Windows environment.
- Experience Working with Diverse Patient Groups: Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
- Case Management Experience: Case management experience or recent related community experience.
- Competency with RAI Assessment Tools: Competent with RAI assessment tools.
- Language Skills: Ability to speak French or another second language.
