About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in coordinating the care and management of palliative care patients in the community.

Key Responsibilities
  • Assess care needs and develop individual care plans for patients with complex needs
  • Link patients with community service providers to maintain their safety and prevent hospital admissions
  • Coordinate and monitor care plan delivery to ensure patients receive high-quality care
  • Establish a helping relationship with patients and their families to support their care needs
  • Act as a subject matter expert for colleagues and external partners on palliative care needs
Requirements
  • Membership in good standing with a regulatory body as a Registered Nurse (RN, BScN)
  • 1+ years of recent experience in community health or a related field
  • Training/certification specific to palliative care
  • Knowledge of EDITH protocol, Symptom Response Kits, and DNR-C paperwork
  • Strong clinical background with a patient-centered approach
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
Preferred Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
  • Case management experience or recent related community experience
  • Competent with RAI assessment tools
  • Ability to speak French or another second language
