Job Summary:

Brightshores is seeking a skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and support to our patients and their families.

Key Responsibilities:

  • Assist with routine activities of patient care under the guidance of an RN and RPN, promoting an environment that is optimal for the physical, social, and emotional well-being of our patients.
  • Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide emotional support and comfort to patients and their families, promoting a sense of calm and well-being.
  • Participate in multidisciplinary teams to ensure seamless care coordination and communication.
  • Maintain accurate and up-to-date patient records, ensuring confidentiality and adherence to Brightshores' policies and procedures.
  • Stay up-to-date with industry developments and best practices in patient care, participating in ongoing education and training to enhance skills and knowledge.

Requirements:

  • Certificate of Completion of the Personal Support Worker Program from a recognized Community College or a combination of relevant experience and education preferably from an acute care facility.
  • Current CPR certification.
  • Recent experience working in a healthcare facility.
  • Successful completion of corporate Code White training (provided by Brightshores).
  • Excellent written and verbal communication skills.
  • Working knowledge of Infection Control Practices.
  • Ability to work collaboratively within a multi-disciplinary team.
  • Excellent organizational and patient assessment skills.
  • Excellent communication and collaboration skills with patients, families, and other healthcare professionals.
  • Excellent problem-solving, decision-making, and interpersonal skills.
  • Commitment to continuing education and professional development.
  • Ability to work all shifts.
  • Recent satisfactory performance and attendance record.
  • Adherence to Brightshores' 'CARE' Behaviours (Collaboration, Accountability, Respect, Excellence) and 'LEAD' Competencies (Lead by example, Empower, Achieve results, Develop others) and a commitment to a safe workplace free of violence and harassment.
  • Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills, and attitudes.
