About the Role:
We are seeking a highly skilled and experienced Care and Service Manager to join our team at ParaMed Home Health Care. As a key member of our leadership team, you will be responsible for leading our clinical supervisors and front-line staff in delivering high-quality care to our patients.
Key Responsibilities:
- Lead and manage a team of clinical supervisors and front-line staff to ensure delivery of high-quality care to patients.
- Develop and implement care plans that meet the unique needs of each patient.
- Monitor and evaluate the quality of care delivered by our team and identify areas for improvement.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide coaching and mentoring to clinical supervisors to enhance their leadership skills and support their professional development.
- Ensure compliance with all relevant laws, regulations, and standards related to healthcare and patient care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in care delivery.
Requirements:
- Registration as a Registered Nurse with a provincial regulatory college.
- Minimum of three years of experience in a formal nursing leadership role.
- Minimum of two years of experience as a practicing Registered Nurse in a community setting.
- Certification in Community Health Nursing with CNA certification preferred.
- Membership with the Registered Nurses Association of Ontario (RNAO) preferred.
- Working knowledge of applicable healthcare legislation and standards.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
