The Occupational Therapist plays a vital role within the interprofessional Dual Diagnosis Consultation Outreach Team (DDCOT) dedicated to serving Southeastern Ontario. This team is responsible for conducting assessments, providing consultations, and delivering short-term therapeutic interventions for individuals aged 16 and older who are facing a dual diagnosis, which includes both an intellectual disability and a diagnosed or suspected mental illness, along with associated behavioral challenges. By employing a collaborative care model, the team partners with community organizations and family physicians to mitigate the effects of mental health issues and enhance the overall quality of life for adults with dual diagnoses and their support networks.
Qualifications and Experience- Bachelor's degree in Occupational Therapy, with a minimum of five years of relevant experience; a Master's Degree in Occupational Therapy is preferred.
- Current, unrestricted registration with the College of Occupational Therapists of Ontario.
- At least three years of experience in dual diagnosis, psychiatry, or a closely related field is preferred.
- Clinical experience with individuals who have a dual diagnosis is advantageous.
- Experience in diverse community settings, including home visits, is essential.
- Demonstrated experience collaborating with families and community organizations.
- Community development experience is a plus.
- Experience in providing case management services is beneficial.
- Conducts thorough assessments, consultations, and treatment for a designated clinical caseload, including follow-up and discharge planning.
- Regularly reviews and assesses the treatment and ongoing care plan in collaboration with clients, caregivers, and care providers.
- Identifies additional support and educational needs for clients, caregivers, and mental health care providers.
- Participates in client-related case conferences as necessary.
- Evaluates risk factors and intervenes appropriately; provides recommendations on equipment and adaptations for home, school, or short-term care settings.
- Monitors psychotherapeutic interventions and assists in tracking pharmacological treatments for effectiveness and side effects.
- Addresses behavioral needs using specialized approaches, including sensory integration techniques.
- Facilitates communication needs and adjusts communication methods to effectively engage with individuals with varying communication abilities.
- Reports significant changes in client status to relevant members of the community health care team, including the client's family physician.
- Maintains documentation of client progress and responses to therapeutic interventions in the client's case file.
