Nurse, Occupational Health and Safety Full-Time

We are seeking a dynamic and passionate Registered Nurse (RN) to join our Transitions team. As a key member of our interdisciplinary care team, you will provide comprehensive care to clients in their homes and communities.
Key Responsibilities- Conduct client assessments and develop individualized care plans based on best practices and client needs.
- Lead the care planning process and work collaboratively with other disciplines to implement client care plans.
- Provide continuous and comprehensive support to clients and their families, addressing physical, cognitive, social, emotional, and spiritual domains.
- Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions.
- Support clients through care transitions by connecting them with community resources and services.
- Minimum of two years of nursing experience.
- Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health.
- Ability to be flexible and adaptable with excellent organizational skills.
- Excellent skills in case management and working with interdisciplinary care teams.
- A vehicle and valid driver's license, with the ability to travel.
