About the Role

We are seeking a highly motivated and detail-oriented individual to join our team as a Social Services Case Manager. In this role, you will work closely with clients and families to provide support and guidance in achieving their goals.

Key Responsibilities
  • Client Support: Provide individualized support to clients and families, including assessment, planning, and implementation of services.
  • Case Management: Manage a caseload of clients, ensuring timely and effective delivery of services.
  • Collaboration: Work collaboratively with internal and external stakeholders, including community agencies and healthcare providers.
  • Documentation: Maintain accurate and up-to-date records of client interactions and service delivery.
Requirements
  • Education: Bachelor's degree in a related field, such as social work or a related field.
  • Experience: Experience working in a social services setting, preferably in a case management role.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
Work Environment
