Dnaagdawenmag Binnoojiiyag Child & Family Services is seeking a skilled Reintegration Worker to join our team. As a Reintegration Worker, you will play a critical role in supporting the connection plans for children and youth in care.
Key Responsibilities- Develop Connection Plans: Work with children and youth to develop personalized connection plans that address their unique needs and goals.
- Implement Connection Plans: Collaborate with service teams to implement connection plans, providing ongoing support and follow-up as needed.
- Accompany Children and Youth: Accompany children and youth to meet with family members, community members, and other identified connections, including air travel and overnight stays.
- Provide Support and Guidance: Offer support, guidance, and share information to ensure the best possible outcomes for children and youth, families, and communities involved.
- Prepare Reports and Statistics: Prepare and maintain regular reports, analyses, and statistics as required.
- Culturally Sensitive Practice: Walk alongside children and youth to support connections that align with their cultural plan, consulting with service workers and Family Finders as needed.
- Reunification Support: Actively work towards the reunification of siblings, supporting visits, establishing contact, letter writing, and social networking.
- Community Engagement: Familiarize children and youth with local Indigenous services and programs, and participate in community and provincial working groups related to the role.
- Case Management: Maintain accurate and up-to-date case files in accordance with agency policies and procedures.
