Job Summary:

This role will provide person-centered Social Work services, working alongside Indigenous clients and their families to assess, intervene, and develop care plans leading to improved wholistic well-being and attainment of goals.

Key Responsibilities:
  • Provide trauma-informed care and use innovative practices to meet the complex needs of clients
  • Collaborate with the specialized interprofessional clinical team, clients, and families to develop and implement tailored psychosocial support plans
  • Maintain a full caseload with support from the Counselling and Wellness Supports (CWS) Team Lead
  • Work in collaboration with interprofessional team members and communities to develop and implement tailored psychosocial support plans
Requirements:
  • Master's degree in social work with a clinical concentration
  • Member in good standing of the Ontario College of Social Workers and Social Service Workers
  • Experience collaborating with Indigenous communities and clients with demonstrated ability to work cross-culturally
  • Experience working in an interprofessional primary care setting
  • Knowledge of a variety of theoretical frameworks and treatment modalities
  • Ability to provide indigenous culturally safe, trauma-informed care and services
  • Ability to understand Indigenous cultural values, traditions, history
  • Ability to integrate Indigenous perspectives into services
  • Knowledge of the Health Care Delivery System and relevant legislation
  • Experience working with determinants of health, primary care social work, chronic conditions, addictions, dying, grief, and loss
  • Ability to provide both group and individual counseling
  • Exceptional knowledge of discharge planning, community resources, and the placement process
  • Excellent assessment and crisis intervention skills
  • Superior written, verbal, and presentation skills
  • Ability to multi-task
  • Superior interpersonal skills
  • Ability to respect individuals' experiences and foster healing
