We are seeking a skilled Social Services Professional to join our Counselling and Wellness Supports (CWS) Team at the Barrie Area Native Advisory Circle. As a member of our team, you will use best practices to provide person-centered Social Work services to Indigenous clients with complex mental, emotional, physical, and spiritual health needs, and their families.
This role will involve assessment, therapeutic intervention, and care planning to improve wholistic well-being and attainment of goals. You will work in collaboration with the specialized interprofessional clinical team, the client, and families to develop and implement tailored psychosocial support plans.
The ideal candidate will be a skilled social worker with strong clinical, advocacy, coordination, and relationship-building skills who is able to provide trauma-informed care and use innovative practices to meet the complex needs of clients.
Key Responsibilities- Provide person-centered Social Work services to Indigenous clients with complex mental, emotional, physical, and spiritual health needs, and their families.
- Assess, develop, and implement individualized care plans to improve wholistic well-being and attainment of goals.
- Collaborate with the specialized interprofessional clinical team, the client, and families to develop and implement tailored psychosocial support plans.
- Provide trauma-informed care and use innovative practices to meet the complex needs of clients.
- Work in collaboration with interprofessional team members and communities to develop and implement psychosocial support plans.
- Master's degree in Social Work with a clinical concentration.
- Member in good standing of the Ontario College of Social Workers and Social Service Workers.
- Experience collaborating with Indigenous communities and clients with demonstrated ability to work cross-culturally.
- Experience working in an interprofessional primary care setting preferred.
- Knowledge of a variety of theoretical frameworks and treatment modalities such as crisis intervention, systems theory, etc.
- Ability to provide Indigenous culturally safe, trauma-informed care and services.
- Ability to understand Indigenous cultural values, traditions, history.
- Ability to integrate Indigenous perspectives into services.
- Knowledge of the Health Care Delivery System and relevant legislation.
- Experience working with determinants of health, primary care social work, chronic conditions, addictions, dying, grief, and loss.
- Ability to provide both group and individual counseling.
- Exceptional knowledge of discharge planning, community resources, and the placement process.
- Excellent assessment and crisis intervention skills.
- Superior written, verbal, and presentation skills.
- Ability to multi-task.
- Superior interpersonal skills.
- Ability to respect individuals' experiences and foster healing.
