Role Summary
The Social Worker II is tasked with providing support to clients and their families facing social and financial challenges associated with continuing care admissions or reintegration into the community. This role involves collaboration with the Community Care Coordinator and Transition Services to facilitate the creation of effective discharge plans. The Social Worker will maintain open communication with clients and their families regarding progress towards suitable discharge arrangements.
Program Overview: The Complex Care program is designed for elderly individuals requiring the expertise of an interdisciplinary team to enhance and sustain their quality of life within safe, comfortable, and supportive settings. Carewest provides a blend of secured and unsecured living environments.
Qualifications & Experience
- Completion of a recognized degree in Social Work or an equivalent qualification.
- Membership with the Alberta Association of Social Workers.
- Possession of a valid Driver's License.
- A minimum of 2 years of experience in clinical and continuing care settings; experience in Gerontology is preferred.
