About the Opportunity
The Peter Lougheed Centre (PLC) is a leading acute care hospital in Calgary, providing both inpatient and outpatient services across various specialties including Medical, Surgical, Pediatrics, Women's Health, and Critical Care. As the Vascular Centre for Southern Alberta, Southwestern Saskatchewan, and Southeastern British Columbia, the PLC plays a vital role in regional healthcare.
The Allied Health Department at the PLC comprises dedicated Social Work, Rehabilitation, and Spiritual Care staff who work collaboratively as integral members of interdisciplinary teams to deliver patient-centered care throughout the hospital.
Your Role
Reporting to the Clinical Leader and Allied Health management, you will provide essential social work services primarily within the Emergency Department. You may also be required to support other acute care units as needed. This dynamic role involves shift work, encompassing days, weekends, and evenings.
Your responsibilities will encompass a range of clinical tasks, including:
- Conducting psychosocial assessments to understand patient needs and challenges
- Identifying and addressing social determinants of health that may impact patient well-being
- Providing crisis intervention and safety planning for patients in distress
- Performing risk assessments to ensure patient and staff safety
- Facilitating system navigation and discharge planning to support smooth transitions
This position demands the ability to work independently in a fast-paced environment while effectively collaborating with the interdisciplinary team. You will adhere to the highest standards of practice as outlined by the ACSW.
Qualifications
• Completion of a bachelor's degree in Social Work
• Active or eligible for registration with the Alberta College of Social Workers (ACSW)
Preferred Qualifications:
- Minimum 2 years of experience as a Social Worker in an acute care setting
- Previous Connect Care training
- Training in motivational interviewing
