Hours: 77.50 FTE: 1.00
Role Summary
This position is dedicated to supporting clients and their families in navigating social and financial challenges associated with ongoing care transitions or reintegration into the community. The role involves close collaboration with the Community Care Coordinator and Transition Services to facilitate the creation of effective discharge plans. It also includes maintaining open communication with clients and their families regarding progress towards suitable discharge arrangements.
Program Focus: The Complex Care program is designed for elderly individuals requiring the expertise of a multidisciplinary team to enhance and sustain their quality of life within secure and nurturing environments. Carewest provides a blend of both secured and unsecured living arrangements.Qualifications & Experience
  • Completion of a recognized degree in Social Work or an equivalent qualification
  • Active registration with the Alberta Association of Social Workers
  • Possession of a valid Driver's License
  • A minimum of 2 years of experience in clinical and continuing care settings; experience in gerontology is highly desirable
