As an Occupational Therapist (Permanent, full-time), your primary responsibility will be to deliver occupational therapy services to patients under the Ontario Health at Home program, aiming to enhance their functionality, independence, and safety within their living environments.

Whether your work setting is an office, community, or healthcare facility, you will be instrumental in providing integrated, accessible, and patient-focused care, supported by a collaborative team of over 9,000 healthcare professionals.

Key Responsibilities:

  • Evaluate the elements affecting the client's ability to function in their surroundings, utilizing various standardized and non-standardized assessment tools.
  • Establish patient-centered therapy objectives and implement treatment plans to enhance occupational performance, independence, and safety.

Collaborate with other healthcare professionals and organizations to ensure effective and efficient patient care delivery.

Foster therapeutic relationships by respecting patients' values and preferences while balancing their needs with available resources.

Maintain accurate documentation of assessment results, goals, and intervention strategies in line with organizational policies and professional standards.

Qualifications:

  • Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
  • Thorough understanding of occupational therapy services and community resources.
  • Comprehensive knowledge of the roles of various healthcare professionals.
  • Relevant experience in occupational therapy within community or healthcare settings.
  • Up-to-date knowledge of occupational therapy tools, methodologies, equipment, and assistive technologies.
  • Familiarity with funding agencies related to occupational therapy recommendations.
  • Understanding of pertinent legislation regarding healthcare service provision, privacy, and safety.
  • Solid grasp of the standards of practice and professional guidelines established by COTO.
  • Ability to prioritize professional responsibilities, manage multiple patients, and organize workload efficiently.
  • Strong interpersonal and collaborative skills to engage with diverse patient groups, case managers, family members, healthcare professionals, and community organizations.
  • Effective listening, observation, and facilitation skills.
  • Proficient communication skills across various formats, including reports and presentations.
  • Working knowledge of computer applications, including Microsoft Office Suite.
  • Valid driver's license and access to a vehicle are required.
  • Advanced proficiency in both English and French is essential.

Preferred Qualifications:

  • Experience working with diverse patient populations and collaborating with case managers, family members, and healthcare professionals.
  • Background in case management within healthcare or community support services.
  • Ability to communicate in French or another second language.
