Job Title: Transitional Housing Worker

We are seeking a skilled and compassionate Transitional Housing Worker to join our team at the Canadian Mental Health Association Toronto Branch. As a key member of our Home for Good Innovative Housing Program, you will play a vital role in providing on-site residential support and case management services to older adults living with mental illness and experiencing homelessness.

Key Responsibilities:

  • Provide direct service provision and support clients based on their goals and needs
  • Orient clients to the program and services, and provide crisis prevention and intervention as needed
  • Maintain a safe, clean, and secure environment in the residential setting
  • Develop and maintain trusting, respectful, and collaborative working relationships with clients, colleagues, and community partners

Requirements:

  • Minimum college diploma in Social Work or related field, or equivalent combination of education and experience
  • Minimum 2 years of experience working with adults with complex mental health and addictions issues, preferably in a community or residential setting
  • Demonstrated knowledge of Concurrent Disorders and the Criminal Justice System as it relates to mental health
  • Strong written and oral communication skills, and ability to work effectively with diverse communities
