Role Overview: The Transitional Housing Support Relief Worker plays a vital role in implementing tailored plans and supportive services for youth, assisting them in achieving their personal goals while residing in the facility. This position also ensures that the youth contribute positively to their environment and the overall well-being of the community.

Key Responsibilities:

  • Provide assistance to Indigenous youth facing mental health and substance use challenges.
  • Conduct ongoing assessments of the mental health and functionality of youth in real-world settings.
  • Maintain youth client files, documenting assessments, incidents, progress, and evaluations.
  • Stay informed about individual care plans and the specific strategies designed to help each youth reach their life goals.
  • Encourage and supervise youth participation in household responsibilities such as cooking and cleaning.
  • Monitor living conditions to ensure safety and well-being.
  • Offer a broad spectrum of support that addresses the physical, mental, spiritual, and emotional needs of youth.
  • Collaborate with the Youth Case Management team to foster a safe and supportive living environment, assisting with education, medical appointments, and life skills.
  • Work with internal and external care teams to develop aftercare plans for youth transitioning out of the program.
  • Implement established support plans and provide crisis intervention as necessary.
  • Partner with community organizations to facilitate access to health and support services.
  • Engage in community referrals and networking.
  • Ensure a clean, safe, and secure environment for all residents and staff.
  • Provide individual and group counseling/support to youth.
  • Perform other related duties as assigned.

Qualifications:

  • Degree or diploma in social work, psychology, counseling, or a related field.
  • At least 2 years of experience supporting vulnerable youth with mental health issues or substance use challenges.
  • Familiarity with community resources and counseling practices relevant to high-risk Indigenous youth.
  • Understanding of the Recovery Model, Harm Reduction, and Anti-Oppressive Framework.
  • Vulnerable Sector Check required.
  • Awareness of cultural sensitivity regarding Indigenous communities.
  • Experience in an on-site housing environment.
  • Ability to establish therapeutic relationships while respecting client autonomy.
  • Capability to perform physical tasks as needed.
  • Strong active listening and interpersonal skills.
  • Proficient crisis intervention and problem-solving abilities.
  • Excellent verbal and written communication skills.
  • Demonstrated leadership and teamwork skills.
  • Empathy and approachability.
  • Experience working with Indigenous and diverse populations.
  • Ability to maintain confidentiality and trust.
  • Proficient in standard office software (e.g., Word, Excel).
  • Strong organizational skills with the ability to manage multiple tasks.

Knowledge, Skills, and Abilities:

  • Commitment to confidentiality and risk minimization.
  • Experience with Indigenous organizations is an asset.
  • Knowledge of Indigenous languages and cultures.
  • Adherence to current health guidelines and regulations.
  • Contribute to data collection and reporting for organizational activities.

Compensation: Salary is commensurate with qualifications and experience. Preference may be given to Indigenous applicants, but all qualified candidates are encouraged to apply.
