Job Summary:

The Transitional Housing Worker will provide on-site direct service provision, including Activities of Daily Living (ADLs) support, group facilitation, social recreational programming, and medication monitoring. You will work closely with clients to build life skills, promote independence, and support their recovery goals.

Responsibilities:

  • Provide on-site support to clients, including ADLs, group facilitation, and social recreational programming
  • Monitor and manage client medication
  • Assess client mental status and provide supportive counseling
  • Develop and implement individualized support plans
  • Collaborate with multidisciplinary teams to ensure comprehensive client care

Requirements:

  • Minimum college diploma in Social Work/Social Sciences, Recreation Therapy, or Life Skills
  • Minimum 3 years of experience working with adults with complex mental health and addiction issues
  • Strong knowledge of psychosocial rehabilitation, recovery principles, and therapeutic treatment modalities
  • Excellent organizational and communication skills
  • Ability to work independently and as part of a multidisciplinary team
