Job Summary:

The Ambulatory Services department has an opening for a Physiotherapist to support various programs. This role involves working creatively in diverse community settings, including outpatient services, day hospitals, outreach teams, and primary care clinics, as well as utilizing digital platforms for virtual care delivery.

Key Responsibilities:

  • Conduct comprehensive clinical evaluations, develop care plans, implement treatment strategies, and oversee discharge planning for patients with complex geriatric, neurological, and musculoskeletal conditions.
  • Engage in the education and mentorship of physiotherapy students and assistants.
  • Oversee the execution of patient care plans, providing guidance and supervision to physiotherapy assistants.
  • Collaborate with an inter-professional team to implement patient-centered care plans and manage discharge processes.
  • Offer support and consultation to team members from other disciplines.
  • Facilitate a systematic, evidence-based approach to client care, program development, and professional practice in partnership with team members and volunteers.

Qualifications:

  • Current licensure with the College of Physiotherapists of Ontario.
  • Membership or eligibility for membership with the Canadian Physiotherapy Association is advantageous.
  • Experience working with geriatric patients with multiple health issues and/or cognitive challenges is preferred.
  • Confirmed status as an ADP authorizer is required.
  • Proficiency in conducting clinical assessments for elderly clients.
  • Ability to lead group sessions and conduct knowledge translation/adult learning activities with clients and team members.
  • Experience in clinical instruction and teaching for physiotherapy students is a plus.
  • Interest in developing evidence-based programs is beneficial.
  • Strong interpersonal, communication, time management, and problem-solving skills.
  • Access to transportation is necessary.
  • Proficient in computer applications including Meditech, MDS, Microsoft Word, PowerPoint, and Excel.
