Job Overview:The Ambulatory Services department is seeking a dedicated Physiotherapist to enhance our team. This role involves supporting various outpatient services, including day hospitals and outreach initiatives. The position requires innovative approaches to care delivery across diverse community settings, including virtual platforms for remote access to services.
Key Responsibilities:
- Perform comprehensive clinical evaluations, develop care plans, and implement treatment strategies for a diverse patient population, including complex geriatric, neurological, and musculoskeletal cases.
- Engage in the education and mentorship of physiotherapy students and assistants.
- Oversee the patient care plan, providing guidance and supervision to physiotherapy assistants.
- Collaborate with an inter-professional team to execute client-centered care plans and manage discharge processes.
- Offer consultation and support to other healthcare professionals within the team.
- Facilitate a systematic, evidence-based approach to patient care, program development, and professional practice.
Qualifications:
- Current licensure with the College of Physiotherapists of Ontario.
- Membership or eligibility for membership with the Canadian Physiotherapy Association is preferred.
- Experience working with geriatric patients with complex medical conditions and cognitive challenges is advantageous.
- Confirmed status as an ADP authorizer is required.
- Proficiency in conducting clinical assessments for elderly clients.
- Ability to lead group sessions and facilitate knowledge transfer among clients, caregivers, and team members.
- Experience in clinical instruction and teaching is a valuable asset.
- Interest in developing evidence-based programs is preferred.
- Strong interpersonal, communication, time management, and problem-solving skills.
- Access to reliable transportation is necessary.
- Proficient in computer applications, including Meditech, MDS, Microsoft Word, PowerPoint, and Excel.
