Job Overview:The Ambulatory Services department is offering an exciting opportunity for a Physiotherapist to become part of our dynamic team. This role will involve providing support to various Ambulatory Services including Day Hospitals and Outreach Teams. The position requires innovative approaches to deliver care in diverse community settings, such as outpatient clinics, day hospitals, outreach teams, and primary care environments, including the use of digital platforms for virtual care delivery.
Key Responsibilities:
- Perform clinical evaluations, develop care plans, execute treatment interventions, and manage discharge planning for patients with complex geriatric, neurological, and musculoskeletal conditions.
- Engage in the education and mentorship of physiotherapy students and assistants.
- Oversee the patient care plan, providing guidance and supervision to physiotherapy assistants.
- Collaborate with an inter-professional team to implement client-centered care plans and coordinate discharge processes.
- Offer support and consultation to other team members.
- Facilitate a systematic, evidence-based approach to client care, program development, and evaluation.
Qualifications:
- Valid licensure with the College of Physiotherapists of Ontario.
- Membership or eligibility for membership with the Canadian Physiotherapy Association is advantageous.
- Experience working with geriatric patients with multiple health conditions and/or cognitive challenges is preferred.
- Confirmed status as an ADP authorizer is required.
- Proficiency in conducting clinical assessments for elderly clients.
- Ability to lead group sessions and facilitate knowledge translation/adult learning for clients and team members.
- Experience in providing clinical instruction to physiotherapy students is beneficial.
- Interest in developing evidence-based programs is a plus.
- Strong interpersonal, communication, time management, and problem-solving skills.
- Access to reliable transportation is necessary.
- Proficient in computer applications including Meditech, MDS, Microsoft Word, PowerPoint, and Excel.
