About the Opportunity

We are seeking a skilled Social Worker to join our team at Nova Scotia Health and IWK Health. As a member of our collaborative interprofessional team, you will play a vital role in delivering high-quality Primary Health Care services to our patients.

The successful candidate will take a participatory approach to screening, assessment, counseling, group facilitation, navigation, care planning, and advocacy. You will work closely with other programs and services, developing extensive relationships and bridging with both internal and external stakeholders.

This is a dynamic role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will be responsible for promoting client-focused care, demonstrating care for and with clients and significant others, and advocating for social justice.

We are looking for a candidate with a Bachelor's Degree in Social Work (BSW) from an accredited and approved School of Social Work/University Program, and current registration (or eligibility for registration) with the Nova Scotia Association of Social Workers.

The ideal candidate will have a minimum of two years of experience in a community-based healthcare setting, with recent experience working with community agencies, resources, volunteers, and support groups. You will also have demonstrated ability to complete initial and ongoing client assessments, interviewing, and client-centered problem-solving, care planning, case management, and evaluation of client competence in self-care and other related outcomes.
