About the Opportunity
- Prepare reports, records, statistics and other data as needed or requested.
- Participates as member of the interdisciplinary team in team, client and family meetings.
- Collaborates with clients and family/friends to meet their program expectations and needs.
- Conduct tours of the facilities and interviews prospective clients and/or families through home, hospital and/or office visits to obtain pre-admission information.
- Coordinates admission, transfer and discharge of clients in coordination with the interdisciplinary team.
- Prepares funding requests and reports relating to client needs; maintains client record as required.
- Promotes safety culture (or act as a safety role model) by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace.
- Ensures client safety by following and applying all standard operational procedures.
About You
- Licensed Practical Nurse Diploma or other applicable education
- CPR certified
- Previous experience working in long term care or with seniors.
- Knowledge of the principles of human behaviour and ability to apply program solving skills.
- Strong computer literacy skills.
- Ability to communicate effectively and efficiently (both verbally and in writing).
- Knowledge of available community program and supports.
- Ability to establish and maintain effective, cooperative working relationships with staff, families, clients and other stakeholders.
