Job Title: Discharge Planner

About the Role:

We are seeking a skilled and compassionate Discharge Planner to join our team at Fraser Health. As a Discharge Planner, you will play a critical role in ensuring a smooth transition of care for our patients from the acute care facility to the community.

Key Responsibilities:

  • Perform client assessments to determine their care needs and facilitate discharge planning from the acute care facility to the appropriate community service.
  • Collaborate with other community service providers to provide advice to clients, families, and healthcare professionals on discharge options.
  • Manage short-term Home Support hours for clients waiting to be discharged or referred to a community service.
  • Arrange client referrals from staff within an assigned acute care facility that requires community care services upon discharge or a change in level of service.
  • Interview and screen clients to gather information and determine their eligibility for community services.
  • Develop and implement client care plans with other members of the interdisciplinary team.
  • Participate in discharge planning rounds in the acute care facility and facilitate timely discharge of patients.
  • Liaise with clients, families, physicians, and other healthcare professionals to arrange for community services.
  • Ensure clients receive access to information about available and eligible services.
  • Facilitate timely exchange and provision of information regarding client care between the acute care facility and community services.
  • Work with community service providers to ensure client care needs are met.
  • Determine if clients are capable of making decisions around their care plans and refer them to appropriate healthcare professionals as needed.
  • Complete and maintain accurate documentation of client information.
  • Participate in quality improvement activities and provide input into the development of indicators, standards, practices, and procedures.
  • Assist in the orientation of new staff and identify self-learning needs.

Qualifications:

  • Graduation from an approved school of Nursing.
  • One year of recent related clinical experience in acute care and/or community nursing and discharge planning.
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Strong knowledge of acute, chronic, rehabilitation, and palliative health conditions.
  • Excellent communication and problem-solving skills.
  • Ability to work independently and as a member of an interdisciplinary team.
  • Physical ability to carry out the duties of the position.
