Community Health Nurse

We are seeking a highly skilled Registered Nurse to join our Community Health Liaison Program at Burnaby Hospital. As a Community Health Nurse, you will work independently in the community setting, collaborating with an interprofessional team to manage client caseloads and provide comprehensive care services.
Key Responsibilities- Establish therapeutic relationships with clients through interpersonal and interviewing techniques, ensuring their autonomy and dignity in decision-making and care planning.
- Screen referrals, conduct individualized client assessments, and develop interprofessional care plans, including clinical care and referral services for clients with complex chronic conditions.
- Develop comprehensive shared patient/client care plans in collaboration with the interprofessional team, primary care provider, client, and family, and facilitate transitions to referring sources and community agencies.
- Provide direct client care, identify required care services, and maintain clinical records, including intake screens, client assessments, and progress notes.
- Participate in department quality improvement and risk management activities, and contribute to the development and revision of standards of care, policies, and procedures.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- One year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
