Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Indigenous Health Liaison Team at Fraser Health. As a key member of our multi-disciplinary team, you will play a vital role in providing culturally appropriate counseling and intermediary services to First Nations, Metis, and Inuit clients.

Key Responsibilities
  • Participate in a variety of community and hospital settings to provide culturally sensitive healthcare services to Indigenous clients.
  • Collaborate with other programs across Fraser Health and Indigenous partner organizations to enhance access to healthcare services.
  • Conduct client assessments and screenings to evaluate emotional, physical, social, spiritual, economic, and mental status.
  • Develop and implement care plans to address the unique needs of Indigenous clients.
  • Provide education and promote health promotion initiatives to increase knowledge and awareness of self-care and whole body health.
  • Act as a client advocate and liaison with community agencies to ensure seamless care transitions.
Requirements
  • Bachelor's degree in Social Work from an approved School of Social Work.
  • Two years' recent, related clinical social work experience in a hospital or community setting, including one year of experience working with Indigenous clients.
  • Knowledge of Indigenous health approaches, protocol, and cultural issues.
  • Current full registration with the BC College of Social Workers.
  • Valid BC Driver's Licence and access to a personal vehicle for business-related purposes.
