Job Summary

We are seeking a highly skilled and compassionate Forensic Community Specialist to join our team at PHSA. As a key member of our multidisciplinary team, you will work closely with individuals with complex mental health and substance use challenges, providing expert nursing care and case coordination services.

Key Responsibilities

  • Conduct comprehensive biopsychosocial assessments, including mental status examinations and physical health evaluations, to inform treatment planning.
  • Develop and implement individualized treatment plans, working collaboratively with clients, families, and other healthcare professionals.
  • Provide case coordination and advocacy services, ensuring seamless transitions between healthcare services and community resources.
  • Maintain accurate and concise documentation, adhering to regulatory requirements and professional standards.
  • Collaborate with multidisciplinary teams to provide comprehensive care, including medication management, counseling, and support services.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two years of recent experience working with individuals with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Driver's License.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
