Job Summary

We are seeking a highly skilled Registered Psychiatric Nurse to join our team at BC Mental Health & Substance Use Services. As a Community Case Coordinator, you will work with individuals with complex mental health and substance use challenges, providing community-based care and support.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans in collaboration with clients, families, and healthcare teams.
  • Provide community-based case coordination and advocacy services, ensuring seamless transitions and continuity of care.
  • Maintain accurate and concise documentation, adhering to regulatory requirements and professional standards.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients.
  • Participate in professional development opportunities to enhance skills and knowledge in forensic mental health and substance use.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two years of recent experience working with individuals with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Driver's License.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
