About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join our team as a Forensic Community Specialist. In this role, you will work with individuals with complex mental health and substance use challenges, providing community-based care and support.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each client.
  • Provide case coordination and advocacy services to ensure seamless transitions between healthcare providers and community resources.
  • Maintain accurate and concise documentation of client information, adhering to regulatory requirements and professional standards.
  • Collaborate with multidisciplinary teams to provide holistic care and support to clients and their families.
  • Develop and maintain relationships with community resources and services to ensure access to necessary support and care.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two (2) years recent, related experience working with individuals with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
