Healthcare Social Worker

We are seeking a highly skilled Social Worker to join our Primary Care team at Northern Health Authority. As a Social Worker, you will play a vital role in providing comprehensive assessment, care planning, and intervention for individuals and families in our community.
Key Responsibilities- Conduct initial and ongoing assessments to identify client needs and develop effective care plans
- Provide direct psychosocial care, counseling, and group work to support clients in achieving their goals
- Collaborate with healthcare providers and the interprofessional team to ensure seamless care coordination
- Develop and maintain relationships with community resources and organizations to support client needs
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Bachelor's Degree in Social Work from an accredited university
- Two years recent related experience in a healthcare setting or a combination of education, training, and experience
- Current registration with the British Columbia College of Social Workers
- Current valid BC driver's license
- Master's Degree in Social Work or a related field
- Experience working with diverse populations, including Indigenous communities
- Knowledge of community resources and services
