Job Summary

We are seeking a highly skilled Social Worker to join our Primary Care team at Northern Health Authority. As a Social Worker, you will play a vital role in providing comprehensive assessment, care planning, and intervention for individuals and families in our community.

Key Responsibilities
  • Conduct initial and ongoing assessments to identify client needs and develop effective care plans
  • Provide direct psychosocial care, counseling, and group work to support clients in achieving their goals
  • Collaborate with healthcare providers and the interprofessional team to ensure seamless care coordination
  • Develop and maintain relationships with community resources and organizations to support client needs
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Bachelor's Degree in Social Work from an accredited university
  • Two years recent related experience in a healthcare setting or a combination of education, training, and experience
  • Current registration with the British Columbia College of Social Workers
  • Current valid BC driver's license
Preferred Qualifications
  • Master's Degree in Social Work or a related field
  • Experience working with diverse populations, including Indigenous communities
  • Knowledge of community resources and services
