**About Us**:
Medical Clinic at Southpointe is a veteran owned and operated integrated health care clinic in the heart of Fort Saskatchewan. We are comprised of a diversified team of Physicians, Nurse Practitioners, a General Surgeon, a Registered Nurse, a Chiropractor, Massage Therapists, a Behavioural Health Consultant and an amazing administration team. Alongside family healthcare and clinical integrated services, we also run weekly Well Women's Clinics for all related women’s health and a Veterans Support program to assist with their complex needs.
**About the Position**:
As a Social Worker, you will be responsible for providing comprehensive social work services to individuals and families in Fort Saskatchewan. Your primary goal will be to empower clients to overcome challenges, improve their overall functioning, and enhance their quality of life. You will work closely with clients from diverse backgrounds, addressing a wide range of social, emotional, and behavioral issues.
**Highlights of the Role**
- Full-time (37.5 hours per week)
- Flexible days, evenings, and weekends
- Fort Saskatchewan, Alberta (25 minutes northeast of downtown Edmonton)
**Key Responsibilities**:
- Conduct assessments to identify client needs, strengths, and resources.
- Develop and implement individualized treatment plans in collaboration with clients and their support networks.
- Provide counseling, advocacy, and support services to clients facing various challenges, including but not limited to mental health issues, substance abuse, family conflicts, and trauma.
- Connect clients with community resources, including healthcare providers, assistance programs, services, and other relevant support systems.
- Monitor client progress and adjust treatment plans as needed to ensure positive outcomes.
- Maintain accurate and confidential client records in accordance with professional standards and organizational policies.
- Collaborate with interdisciplinary teams and external agencies to coordinate services and maximize client support.

**Qualifications**:
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Registered Social Worker (RSW) designation with the Alberta College of Social Workers (ACSW) or eligibility for registration.
- Minimum of 2 years of experience in social work or related field.
- Experience working with Veterans Affairs is preferred.
- Knowledge of local resources and support services in Fort Saskatchewan and surrounding areas.
- Valid driver's license and access to a reliable vehicle.
