Primary Care Social Worker

We are seeking a highly skilled and dedicated Primary Care Social Worker to join our team at Northern Health Authority. As a member of our interprofessional team, you will play a vital role in providing comprehensive assessment, care planning, and intervention for individuals and their families.
Key Responsibilities- Conduct initial and ongoing assessments to identify client needs and develop care plans
- Provide counseling and group work to individuals and families
- Facilitate client self-management and linkage to relevant resources
- Collaborate with healthcare providers and the interprofessional team to provide comprehensive care
- Develop and maintain relationships with community resources and organizations
- Bachelor's Degree in Social Work from an accredited university
- Two years recent related experience in a healthcare setting
- Current registration with the British Columbia College of Social Workers
- Current valid BC driver's license
- Assessment and Treatment:
- Demonstrated ability to complete initial and ongoing assessments
- Utilize appropriate social work interventions
- Teaching:
- Ability to teach and coach one-on-one and in groups
- Knowledge Integration:
- Integrates best practice and current research evidence to support professional practice decisions and actions
- Communication:
- Demonstrated ability to communicate effectively with individuals, families, and healthcare providers
- Critical Thinking:
- Problem solves demonstrating critical thinking and decision-making skills
- Human Caring and Relationship Centered Practice:
- Ability to promote person-centered care and maintain sensitivity to diverse cultures and preferences
- Management:
- Demonstrated ability to organize work, set objectives, and priorities
- Leadership:
- Demonstrated ability to promote teamwork and a commitment to excellence in the provision of person-centered care
- Teamwork:
- Demonstrated ability to foster teamwork and collaboration across disciplines
- Equipment:
- Demonstrated computer skills and ability to operate other related equipment as required
- Ability:
- Physical ability to perform the duties of the position
