Primary Care Social Worker

We are seeking a highly skilled and dedicated Primary Care Social Worker to join our team at Northern Health Authority.
Job Summary- Provide direct psychosocial care, counseling, and group work to individuals and their families.
- Facilitate client self-management and linkage to relevant resources.
- Collaborate with healthcare providers and the interprofessional team to provide comprehensive care.
- Maintain or improve the health and functioning of individuals, families, and communities.
- Conduct initial and ongoing assessments to identify client needs and develop care plans.
- Provide counseling and group work to individuals and families.
- Facilitate client self-management and linkage to relevant resources.
- Collaborate with healthcare providers and the interprofessional team to provide comprehensive care.
- Develop and maintain relationships with community resources and organizations.
- Bachelor's Degree in Social Work from an accredited university.
- Two years recent related experience in a healthcare setting.
- Current registration with the British Columbia College of Social Workers.
- Current valid BC driver's license.
