Job Summary:
We are seeking a highly motivated and compassionate Community Support Specialist to join our team at the ALS Society of Canada. As a Community Support Specialist, you will work closely with stakeholders across Central, North Simcoe Muskoka and Northern Ontario to provide support services for people living with ALS and their families.
Key Responsibilities:
- Manage a client caseload of approximately clients by establishing regular communication with people living with ALS and their families to provide support and help them navigate the healthcare system.
- Identify issues, manage information, and provide non-clinical support to clients and families.
- Facilitate support groups, make referrals to community support services, bereavement services as needed.
- Liaise with healthcare and social services professionals to support client care and advocate on their behalf when required.
- Raise awareness about ALS disease to healthcare providers, community agencies, service clubs, students, families, and the public.
- Develop partnerships and relationships with healthcare and social services organizations.
- Represent ALS Canada in local and provincial advocacy efforts, including community events, media interviews, speaking engagements, and presentations.
- Assist with Walk to End ALS events, working with the Fund Development Team to drive recruitment and training for event day Walk volunteers, manage all planning, sponsorship, relationships, promotion, and media during walk preparations.
- Participate in and develop fundraising community events in the region as required and attend events as an ambassador of ALS Canada.
- Work independently from a home office.
Requirements:
- Successful completion of an accredited health discipline program and eligible for registration with a professional regulatory college (e.g., Social Work or relevant field).
- ~3-5 years' experience in a healthcare-related field.
- ~Excellent computer skills (Word, Excel, PowerPoint) and experience with CRM systems.
- ~Demonstrated excellence in relationship building, working as part of a team, problem-solving, advocacy skills, and time management.
- ~Comfortable connecting with clients, teammates on a virtual platform.
- ~A valid driver's license and access to a vehicle.
- ~Ability to work flexible hours, including evenings and weekends.
- ~Experience with palliative care is an asset.
- ~Proven fundraising track record is an asset.
- ~Bilingualism is an asset.
