Resident Care Coordinator

We are seeking a highly skilled Resident/Client Care Coordinator to join our team at Queens Park Care Center in New Westminster, BC. This is a Relief Part Time opportunity available until July 2025 or until Return of Incumbent.
About the RoleThe Resident/Client Care Coordinator will provide clinical supervision, guidance, and support to nursing and nursing support staff, and coordinate clinical team members within the assigned unit(s). This role is responsible for the day-to-day operations of the assigned area(s) and supports/participates in change management, quality improvement, risk management, policy, program, research, and/or regional activities.
Key Responsibilities- Develop and implement individual care plans to meet the ongoing needs of residents/clients, in collaboration with other clinical team members and residents'/clients' families.
- Supervise nursing and nursing support staff, observe work, and correct where necessary, assign and adjust work assignments, schedule staff, monitor and recommend staff hours of work, including overtime.
- Support quality resident/client care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out resident/client/family education regarding chronic illness, palliative care, etc.
- Participate in or lead the investigation of complaints by residents/clients/families, follow up on complaints, and implement corrective action as appropriate.
- Initiate/lead/support plan of care conferences and rounds for the purpose of optimal resident/client care management according to best practice.
- Identify learning requirements of staff and provide input into the development of learning objectives, and contribute to the achievement of learning objectives.
- Participate in change management, quality improvement, and risk management activities by evaluating the effect of changes in practice, generating recommendations for alternative approaches to practices, and conducting safety audits.
- Assist the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resources.
- Promote staff awareness and involvement in research/evaluation activities and projects, and participate in research and projects as appropriate.
- Collaborate with internal and external stakeholders, participate on a variety of designated professional or operational committees at the unit, site, program, and/or regional level.
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training, and experience. Certification applicable to the designated clinical area.
