BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Forensic Community Specialist to join our team. As a key member of our multidisciplinary team, you will work closely with individuals with complex mental health and substance use challenges, providing expert care and support to help them achieve their goals.
Key Responsibilities:- Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each client.
- Provide expert nursing services, including medication management, counseling, and education to promote optimal health outcomes.
- Collaborate with community resources and healthcare providers to ensure seamless care coordination and access to necessary services.
- Develop and maintain strong relationships with clients, families, and community partners to promote trust and understanding.
- Participate in ongoing education and training to stay current with best practices and evidence-based treatments.
- Diploma in Nursing or Psychiatric Nursing.
- Two (2) years recent, related experience working with individuals with severe and persistent psychiatric illness.
- Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
- Valid BC Drivers License.
- Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
