Job Title: Forensic Community Specialist

BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Forensic Community Specialist to join our team. As a key member of our multidisciplinary team, you will work closely with individuals with complex mental health and substance use challenges, providing expert care and support to help them achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each client.
  • Provide expert nursing care and support to clients, including medication management, counseling, and education on mental health and substance use.
  • Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and other healthcare professionals, to ensure comprehensive care and support.
  • Develop and maintain relationships with community resources and services to ensure seamless transitions and ongoing support for clients.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance client care and outcomes.
Requirements:
  • Diploma in Nursing or Psychiatric Nursing.
  • Two (2) years recent, related experience working with individuals with complex mental health and substance use challenges.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
