Our Edmonton Zone Continuing Care offers a range of services across five primary program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are seeking a Registered Nurse to become a vital member of our Continuing Care Access team, providing essential support services to clients of all ages and backgrounds.

If you are passionate about connecting individuals in the community with both internal and external programs and services, this role may be an excellent fit for you. Utilizing strong tele-triage skills, you will assess, identify, and clarify client needs, triaging referrals to the appropriate agencies and service partners. You will also provide after-hours telephone support to current Continuing Care clients, monitoring their health status in a dynamic work environment that values teamwork and promotes professional growth.

Role Overview:

As a Registered Nurse (RN), you will deliver a comprehensive range of nursing services to patients, families, and communities, ensuring their safety and well-being. Your responsibilities will include:

  • Employing nursing processes through critical thinking, problem-solving, and decision-making.
  • Teaching, counseling, and advocating for patients and their families.
  • Playing a leadership role in promoting an integrated and holistic approach to patient care, health promotion, and maintenance.
  • Providing safe, quality, patient-centered care while embodying the shared vision and values of AHS.

Please note that Provisional Permit Holders will receive a specific pay rate, which will be adjusted upon registration as a Registered Nurse in accordance with AHS/UNA guidelines. This position may be used to fill multiple identical vacancies arising from the administration of the Trial of Modified Posting Provisions.

Required Qualifications:

To qualify for this position, you must have:

  • Completion of an accredited nursing education program.
  • Active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

If you have not practiced as a Registered Nurse within the last five years, completion of the following free courses is required: CRNA Medication Management - Chapters and the NextGenU Health Assessment Resources Certification.

Additional Qualifications:

Preferred qualifications include:

  • Baccalaureate degree in nursing (preferred), though relevant skills and experience in community nursing will be considered.
  • Palliative care experience.
  • Previous experience in Continuing Care and/or geriatric nursing.
  • Familiarity with Meditech, Connect Care, and the RAI Home Care assessment tool.
  • A minimum of two years of current Case Management experience in Continuing Care.
  • Experience with advanced telecommunications systems is also advantageous.
