About the Role

We are seeking a compassionate and dedicated Personal Support Worker to join our team at DTOC III Long Term Care LP. As a Personal Support Worker, you will play a vital role in providing high-quality care and support to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Provide Personal Care and Support: Assist residents with daily living activities, such as bathing, dressing, and grooming, while promoting their independence and dignity.
  • Monitor and Report Changes: Observe and report any changes in a resident's physical, emotional, or behavioral status to registered staff, ensuring timely interventions and adjustments to their care plan.
  • Document Resident Care: Accurately and thoroughly document resident care, including activities of daily living, in electronic health records, adhering to relevant policies and procedures.
  • Collaborate with Interdisciplinary Team: Work closely with the nursing team, therapists, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident.
  • Ensure Resident Safety and Well-being: Implement and follow precautionary and preventative measures to ensure resident safety, such as skin impairment, falls, and expressive behaviors, as outlined in their care plan.
  • Communicate Effectively: Maintain effective and professional communication with residents, families, and colleagues, promoting a positive and respectful environment.
Requirements
  • Education and Certification: Possess a Personal Support Worker (PSW) certification from a program that meets the Ministry of Colleges and Universities' requirements for an Ontario postsecondary institution or district school board to issue a PSW certificate.
  • Experience and Skills: Have experience working in a long-term care setting, with a strong understanding of person-centered care philosophy and models. Possess excellent communication, interpersonal, and problem-solving skills.
  • Additional Requirements: Provide a current Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one. Submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements. COVID-19 vaccination is a condition of employment.
