Job Summary:

At Brightshores, we are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities:

  • Assist with routine activities of patient care under the guidance of a Registered Nurse (RN) and Registered Practical Nurse (RPN).
  • Collaborate with healthcare professionals to promote a safe and supportive environment for patients.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care delivery.
  • Participate in ongoing education and training to enhance knowledge and skills in patient care and support.
  • Contribute to a culture of patient safety and quality care within Brightshores.

Requirements:

  • Certificate of Completion of the Personal Support Worker Program from a recognized Community College or equivalent experience and education.
  • Current CPR certification.
  • Recent experience working in a healthcare facility.
  • Successful completion of corporate Code White training.
  • Excellent written and verbal communication skills.
  • Working knowledge of Infection Control Practices.
  • Ability to work collaboratively within a multi-disciplinary team.
  • Excellent organizational and patient assessment skills.
  • Commitment to continuing education and professional development.
  • Ability to work all shifts.
  • Recent satisfactory performance and attendance record.
  • Adherence to Brightshores' CARE Behaviours and LEAD Competencies.
