Patient Care Coordinator – pt Health

Employment Type: Permanent Part Time

The Patient Care Coordinator plays a vital role in establishing and maintaining a strong connection between the clinic and its clientele, which includes patients, staff, funding agencies, and referral partners. This is accomplished by delivering efficient administrative assistance and coordinating clinic operations with a friendly, enthusiastic, and professional demeanor towards all patients, team members, and external stakeholders.

The Patient Care Coordinator is tasked with managing all facets of patient intake, data management, responding to phone inquiries, making outbound calls, handling faxes and emails, as well as processing invoices and payments.

Key Responsibilities

Respond to telephone inquiries, welcome patients and visitors upon arrival, and ensure exceptional customer service for all inquiries or issues Facilitate a thorough and precise intake of information during the patient's initial visit to the clinic and throughout their treatment journey Assist patients in completing necessary documentation at the clinic to minimize errors and ensure accurate information Organize the scheduling of patient appointments and maintain comprehensive medical records Collect payments at the time of service delivery Monitor coverage for all funding sources to guarantee approval Execute data entry and billing tasks Manage courier services (incoming and outgoing) and process all email, mail, and fax communications Provide administrative support to the Care Coordinator Manager and Clinic Director

Qualifications & Core Competencies

High School Diploma, G.E.D. or equivalent qualification Strong customer service abilities and telephone communication skills High level of organizational skills with the capacity to prioritize and manage multiple tasks Excellent verbal and written communication skills along with interpersonal abilities Proficient computer skills and data entry capabilities Adaptable and flexible in response to change Previous experience in a Physiotherapy clinic is advantageous
