Community Support Specialist

Job Summary
The Community Support Counselor – Addiction Specialist will participate as a member of the Capacity Building Team at St. Joseph's Healthcare Hamilton. This role will provide expertise in consultation for patients with concurrent disorders and addictions across in-patient and out-patient programs within all medical and mental health and addictions programs.
Key Responsibilities
- Provide support to the interdisciplinary team in utilizing evidence-based treatment for addictions/concurrent disorders
- Demonstrate advanced clinical reasoning skills
- Work in partnership with the Concurrent Disorder outpatient program and the RAAM Clinic
- Participate in the development and implementation of treatment plans for patients with complex addiction/medical issues
- Provide clinical support to patients transitioning from hospital to community
- Collaborate with multidisciplinary teams to provide comprehensive care
Requirements
- Bachelor's Degree in Psychology, Sociology, or relevant discipline
- Evidence of ongoing advanced addictions education from an accredited educational institution
- Registration with the college of psychotherapy (preferred)
- Two years recent related experience working in a mental health setting utilizing an evidence-informed concurrent disorder framework
- One year specific experience in an addiction treatment or medical setting (preferred)
- Current First Aid/CPR certification
- CPI-Crisis Prevention Training (or successful completion as part of clinical orientation)
- Demonstrated knowledge of relevant legislation, clinical psychiatry, and psychiatric medications
- Demonstrated competence in evidence-based addiction individual and group-based counseling techniques and interventions
- Excellent communication, interpersonal, and problem-solving skills
