Job Summary:
The Community Support Specialist - Peer Support Counselor participates as a member of a multidisciplinary team to provide assessment, treatment, rehabilitation support, skills teaching and clinical management in clients' environment of choice for individuals with severe and persistent mental illness. The Community Support Specialist - Peer Support Counselor also provides education, support, and consultation to families along with crisis intervention support.
Key Responsibilities:
- Participate as a member of a multidisciplinary team to provide assessment, treatment, rehabilitation support, skills teaching and clinical management in clients' environment of choice.
- Provide education, support, and consultation to families along with crisis intervention support.
- Act as a role model, promoting hope and empowerment in clients' recovery and enhances team engagement with clients.
- Develop and implement individualized treatment plans that address the unique needs and goals of each client.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Provide support and guidance to clients and their families to help them navigate the mental health system.
- Participate in ongoing education and training to stay current with best practices and research in the field of mental health.
Requirements:
- Post-secondary education in a related field, such as psychology, social work, or counseling.
- Minimum 3 years of experience working in a mental health setting, preferably in a community-based setting.
- Significant lived personal experience with mental illness and recovery.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual respect.
- Strong assessment and problem-solving skills, with the ability to think critically and creatively.
- Knowledge of mental health legislation and policy, as well as community resources and services.
- Valid driver's license and access to a reliable vehicle.
Working Conditions:
The Community Support Specialist - Peer Support Counselor will work in a fast-paced, dynamic environment, providing services to clients in their homes, community settings, and other locations as needed. The position requires flexibility, adaptability, and a strong commitment to providing high-quality services to clients and their families.
