Job Summary:

The Community Support Specialist - Peer Support Counselor participates as a member of a multidisciplinary team to provide assessment, treatment, rehabilitation support, skills teaching and clinical management in clients' environment of choice for individuals with severe and persistent mental illness. The Community Support Specialist - Peer Support Counselor also provides education, support, and consultation to families along with crisis intervention support.

Key Responsibilities:

  • Participate as a member of a multidisciplinary team to provide assessment, treatment, rehabilitation support, skills teaching and clinical management in clients' environment of choice.
  • Provide education, support, and consultation to families along with crisis intervention support.
  • Act as a role model, promoting hope and empowerment in clients' recovery and enhances team engagement with clients.
  • Develop and implement individualized treatment plans that address the unique needs and goals of each client.
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
  • Provide support and guidance to clients and their families to help them navigate the mental health system.
  • Participate in ongoing education and training to stay current on best practices and research in the field of mental health.

Requirements:

  • Post-secondary education in a related field, such as psychology, social work, or counseling.
  • Minimum 3 years of experience working in a mental health setting, preferably in a community-based setting.
  • Significant lived personal experience with mental illness and recovery.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
  • Strong problem-solving and crisis intervention skills, with the ability to think critically and make sound judgments.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Valid driver's license and access to a reliable vehicle, with adequate insurance.

The Community Support Specialist - Peer Support Counselor will work in a fast-paced, dynamic environment, providing services to clients in their homes, community settings, and other locations as needed. The position requires flexibility, adaptability, and a strong commitment to providing high-quality services to clients and their families.
