Key Responsibilities:- Develop and implement mental health programs that cater to the needs of residents in the long-term care community.
- Collaborate with leadership and team members to ensure programs align with Sienna's mission, vision, and values.
- Provide consultation, assessment, development, implementation, and evaluation of individual plans of care.
- Conduct rounds to ensure plans of care meet the needs of residents.
- Registered Nurse, Advanced Practice Nursing, or Regulated Health Professional in a relevant field.
- 5 years of clinical experience involving clients with mental health and addiction issues.
- Design experience in developing programs using evidence-based practices.
- Experience in leading change, monitoring, and measuring outcomes, and providing advanced clinical care.
- Excellent interpersonal, communication, negotiation, teaching, and computer skills.
