We are seeking a skilled and compassionate Healthcare Support Worker II to join our team at Fraser Health. As a key member of our Home Health team, you will provide essential support to clients in their own homes, promoting independence and well-being.
Key Responsibilities- Assist clients with daily living activities, such as bathing, dressing, and grooming, to promote maximum independence.
- Provide personal care, housekeeping, and home management services, as outlined in the client's care plan.
- Perform delegated tasks, such as catheter care and exercise routines, under the guidance of professional staff.
- Conduct housekeeping tasks, including cleaning, laundry, and maintenance, to ensure a safe and healthy environment.
- Collaborate with professional staff to identify and address client needs, reporting any changes or concerns.
- Provide education and support to clients and their families on personal care, housekeeping, and home management skills.
- Monitor and record client food intake, as per care plan instructions.
- Observe and report changes in client behavior, appearance, or living arrangements to professional staff.
- Maintain confidentiality and adhere to established policies and procedures.
- Participate in quality improvement activities and attend in-services and staff development meetings as required.
- Grade 12, plus a Home Support/Resident Care Aide certificate, or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
